Transitional Care Center
As inpatient hospital stays get shorter, transitional care is a way to give patients who need it, an extension of hospital care. Clarion Hospital's Transitional Care Center (TCC) is designed for patients who are recovering from hospitalization, but are not well enough to manage by themselves at home.
TCC residents stay an average of 10-15 days. Located right in the hospital, the TCC gives residents access to daily physical and occupational therapy and nursing care. A team of therapists, nurses, physicians and other professional staff design an individual care plan for each resident. Families are strongly encouraged to be a part of this team so that they, too, can help work towards their loved one's recovery. You may reach the TCC by calling (814) 226-1645
