CLARION HOSPITAL’S TRANSITIONAL CARE CENTER “TRANSITIONS” TO NEW SERVICE On June 15, the Transitional Care Center at Clarion Hospital will cease operation, and on July 1, it will re-open as an inpatient rehabilitation unit, called The Rehab Center at Clarion Hospital. This transition will enable Clarion Hospital to care for those suffering from compromised mobility due to acute illness.
The Transitional Care Center (TCC) was relocated to the hospital’s first floor earlier this year in order to prepare for this change.
The staffing of the inpatient rehab unit will remain the same as that of the TCC. Wendy Haws, R.N., B.C., will continue to manage the department, which will remain under the direction of Thomas Freenock, M.D.
“Our community has come to love our TCC because of the great nurses that work here,” said Haws. “Many patients, who were candidates for transitional care will be candidates for inpatient rehab. And, we want them to know and feel confident that they’ll receive the same excellent, compassionate care.”
Candidates for inpatient rehab include people who have had:
- Broken hips
- A vehicle accident
- A stroke
- Bilateral joint replacements (e.g., both knees replaced)
- Amputation
- Brain injury
- Illnesses that cause complications in MS or Parkinson’s patients
The staff at the inpatient rehab, along with the referring physician, will determine if placement is appropriate.
