Patient Registration 
 In order to make the registration process go as quicklt and hassle-free as possible, all patients should bring the following with them when coming to Clarion Hospital for any outpatient testing:
- Physician's Order - If your physician gave you a written order for testing, please bring the order with you. If your physician was phoning or faxing the order to the hospital, please call (814) 226-3436 or 1-800-522-0505 ext. 3436.
- Physician's Office Contact Information - If your physician is not in the Clarion area, please bring the name, telephone number, and address of his or her office so that the results of your testing can be sent to the proper location.
- Insurance Card(s) - Please be sure to have your current health insurance card(s) with you and available for registration personnel to copy.
- Referral(s) and/or Authorization(s) - If your health insurance provider requires you to have a referral or authorization for the testing that you are registering for, please bring them with you.
