While life has given you a difficult situation, we want you to be assured the Rehab Center at Clarion Hospital is here to help you return to the highest level of independence possible. We will keep you comfortable in a private room, while giving you just the right amount of exercise in our therapy suite.

Your Rehab Team

You will be under the medical supervision of Thomas J. Freenock, Jr., M.D. A board-certified physiatrist with decades of experience, Dr. Freenock specializes in muskulo-skeletal medicine. Dr. Freenock will develop a treatment plan that’s specific for your needs.

Your nurses have been specially trained to care for patients in rehab and will stay with you around the clock to give you and your family all the attention you deserve.

Your physical therapists will teach you exercises to strengthen your bones and muscles. If necessary, they will train you to safely use assistive equipment like canes, walkers, or prosthetic devices.

Your occupational therapists will help restore your ability to do daily tasks, like bathing, eating, and household chores.

Your speech therapists will help you if your ability to speak has been compromised or if you’re having trouble swallowing.

Your case manager and social worker will help you and your family understand your physical abilities and what changes have and will take place. They will also coordinate your stay and discharge.

Other people may play a role in your care, like a registered dietitian to help you with special nutritional needs, laboratory technicians who test your blood to make sure your health is on track, respiratory therapists who will help with breathing difficulties, and so forth.

Your Stay With Us

Every patient has different requirements, and your stay with us will begin with an assessment of your own unique needs. Your therapy will begin as early as your first day, with sessions at least five days a week. We want you to stay as active as possible; therefore, there will be group activities with other patients. Since your willingness and positive attitude will ensure the greatest recovery possible, we encourage your family to participate in your treatment. They are not only of great moral support, but they can benefit from the family training provided by our professional staff. Before your scheduled discharge, your case manager will meet with you and your family for any further explanation of your continued care. You may need ongoing therapy at an outpatient rehab center or in your home. Your case worker will follow up with you to make sure your individual goals are met.

What To Bring

· Three changes of clothes. We suggest comfortable pants or shorts.

· Sneakers

· Night clothes

· Robe

· Sweater

· Cosmetics and other personal toiletries

· Glasses, hearing aids, and other personal items

· Insurance cards

· Books, knitting, or other things to fill your time

Do not bring valuables, such as money or jewelry.
